HealthPoint was established in 2013 in accordance with the Centers for Medicare & Medicaid Services (CMS), and Department of Health & Human Services (HHS) under the Affordable Care Act.
The Medicare Shared Savings Program was established by the Affordable Care Act which allows the creation and participation in Accountable Care Organizations (ACOs) such as HealthPoint to facilitate the coordination of care for Medicare Fee-for-Service beneficiaries between hospitals, health care providers, doctors, and other health care professionals. This coordinated care is designed to create efficiencies and provide Medicare Fee-for-Service patients with high quality service and care, while reducing Medicare costs.
HealthPoint is committed to working closely with Medicare
and other federal agencies across the board, participating healthcare providers, hospitals and your doctor to ensure you receive a high quality of healthcare.
